The Mother Baby Unit

The Mother Baby Unit is staffed with civilian and military staff specially trained to care for postpartum mothers and their babies. The mother and baby stay together in one room throughout the hospital stay, which normally lasts 24 – 72 hours.

The unit is locked at all times and you will need to call the staff from the unit door to request entrance.  

Planning for a child is a fun and exciting time in your life. Once you’ve delivered, it is important that you take the necessary steps to apply for their birth certificate, social security card, and they get enrolled in the Defense Enrollment Eligibility Reporting System (DEERS) and TRICARE.

Getting Your Child’s Birth Certificate

In Texas, the hospital administrator or a designee acting as a midwife is responsible for filing the birth certificate when giving birth in a hospital. This means that the hospital staff will handle the process of obtaining and submitting the birth certificate to the appropriate authorities.
You can get vital records in the county, city, or district where the event occurred. Some offices also provide birth records from anywhere in the state.  Counties, cities, and districts set their own hours of operation, fees, and procedures for requests.
** Ensure you request the Long Form Birth Certificates - Long form birth certificates are more detailed with the time of birth, hospital of birth, doctor information, and parents information.  Used most often to obtain a passport and typically required for purposes of dual citizenship, Indian Registry and immigration. Because this format contains information that can facilitate identify theft, the State recommends that you order this format only when it is required.**
Delivery at the Hospital: Follow the instructions below on how to Apply for Your Child’s Birth Certificate.
Delivery other than the Hospital: To obtain a birth certificate for a delivery other than the hospital, gather the required documents and following the instructions below on how to Apply for Your Child’s Birth Certificate.
Required Documentation

  • Proof infant was born alive - Infants early checkup will work for this displaying date of birth or EMS/ambulance report if applicable, or statement from a medical physician
  • For EMS Report if baby was delivered off Fort Hood

  • For EMS Report if baby was delivered on Fort Hood
  • Go to CRDAMC Release of Information
  • Proof of Pregnancy- Prenatal records or statement from physician. CRDAMC Labor & Delivery/Birth Office will assist.
  • Proof that the birth occurred in the registration district - Utility bill, rent receipt, envelope with address of birth or identification (driver’s license) displaying address of home birth location will be sufficient
  • Proof that the infant's birth occurred on the date stated- An affidavit (notarized)* presented from one person along with photocopy of ID (other than the parents) having knowledge of the pregnancy/birth. This can be in writing on any type of paper and then notarized. An EMS if applicable is sufficient for this proof as well.
  • Parent’s photo Identification – Identification of both parent’s
  • An Acknowledgment of Paternity (AOP) if applicable – is a legal document that allows parents who are not married to establish legal paternity.  If the parents wish, contact CRDAMC Birth Office to schedule an appointment to create this legal document.

Getting Your Child Enrolled in the Defense Enrollment Eligibility Reporting System (DEERS) and TRICARE

You have 90 days from your child's birth, adoption, or court appointment to register the child in the Defense Enrollment Eligibility Reporting System and enroll the child or make any eligible enrollment changes.
  • Sponsors are automatically registered in DEERS.
  • Sponsors must register eligible family members.
If both parents are service members, choose one to be listed as each child’s sponsor in DEERS.

Applying for Your Child’s Social Security Card

At the Hospital (Recommended)
  1. Check "Yes": When filling out your baby's birth certificate paperwork at the hospital, say "yes" to applying for a Social Security number.
  2. Provide Info: You'll need to give both parents' SSNs.
  3. Automatic Process: The hospital sends your baby's birth info electronically to the SSA, and they mail the card to you.
At a Social Security Office (If You Missed the Hospital)
  1. Fill Form SS-5: Complete the Application for a Social Security Card (Form SS-5).
  2. Gather Documents: You need original documents:
  3. For the Baby: Birth Certificate (proof of age, citizenship) & Hospital Birth Record (proof of identity/age).
  4. For You: Your Driver's License/Passport (proof of identity).
  5. Submit: Take these to your local Social Security office; you must make an appointment.
