Family Members & Retirees
Accessing care with the CRDAMC health system is easy. We have eight patient centered medical homes (PCMH) for Family Members and Retirees. You will receive most of your health care at your assigned PCMH. If you need specialty care your primary care manager will make a referral to the appropriate specialty clinic. You may be referred to a specialty clinic at CRDAMC or to a TRICARE Network Provider based on your medical needs and availability.
Patient Centered Medical Homes provide the following services:
- Acute Medical Care
- Routine Medical Care
- Well Baby, Well Child Exams
- Well Woman Exams
- Physicals
- Minor Injury
- Chronic Illness Management (limited)
