Soldier Medical Readiness Center

The Soldier Medical Readiness Center (SMRC) provides military schools physicals and separation/retirement physical exams.

All physicals consist of two parts:

• Part 1 includes labs, x-ray, hearing test, and vision test. If you wear glasses, please bring them to your appointment; do not wear contacts.
• Part 2 will not be scheduled without the completion of part 1. Service members are to wear the PT uniform and arrive 15 minutes early.

Separation/Retirement Physicals

Who needs a Separation History and Physical Exam (SHPE)?

• All Active Duty (AD) and Active Guard and Reserves (AGR) Service Members
• Reserve Component (RC) on AD orders for 180 days or more of continuous service
• RC on AD orders for 31 days or more in support of a contingency operation

• Request medical records from CRDAMC medical records. This may take some time.
• If filing a VA claim, begin preparing for the Benefits Delivery at Discharge (BDD) program claim process at Benefits Delivery at Discharge Program - Veterans Benefits Administration (va.gov).

• If not already done, request medical records from CRDAMC medical records.
• Filing a VA claim: The SHPE is not required by the VA for filing a claim. Visit the VA website for up-to-date information at Benefits Delivery at Discharge Program - Veterans Benefits Administration (va.gov).
• Not filing a VA claim: Nothing else to do once medical records have been requested.
• Undecided: There is time after discharge to file a claim; medical records are required. Visit the VA website at VA Disability Compensation | Veterans Affairs for up-to-date information.

• Schedule part 1 SHPE appointment.
• Complete DD Forms 2697, 2807-1, and 2808 at Medical Operations Data System (MODS).

• Preference is to complete part 2 of the SHPE less than 30 days from separation/retirement date or start of terminal leave date, whichever is earliest. The SHPE can be completed during terminal leave. The SHPE cannot be completed by SMRC after the SM has been separated/retired.
• If the SHPE has been completed greater than 30 days prior to separation/retirement date or start of terminal leave, the physical will need to be validated utilizing DD Form 2697 by your TMC provider.
• If your BDD claim has been completed and the exams are in the electronic health record (in JLV), the VA Separation Health Assessment (VA-SHA) or “general exam” can be validated by SMRC or your TMC provider and does not require the full SHPE.

If reporting for a Chapter physical please bring the memorandum requesting the chapter and escort, if required.

How to Make an Appointment

Part 1 physical appointments can be made through:

• MHS Genesis Patient Portal at DS Logon - DMDC (osd.mil)
• Email at dha.cavazos.darnall-amc.mbx.smrc-staff@health.mil with the following information:
  • Name, DOB, and DODID number
  • Type of physical requesting
  • Separation date and final out or terminal leave date (if applicable)

Part 2 physical appointments are scheduled at the completion of part 1.
All Special Operations physicals must be scheduled in person at SMRC.
All flight physicals must be done at Russell Collier Health Clinic by appointment only at 254-553-3001.
SMs not enrolled to CRDAMC (Cadets, Guard, Reserves) must register first with Patient Administration at 254-288-8171.
