Soldier Medical Readiness Center
The Soldier Medical Readiness Center (SMRC) provides military schools physical exams, and separation physical exams (SHPE) for ETS, retirement, and administrative separation (chapter).
SMRC does not do PHAs. Please contact your TMC to schedule.

Separation History and Physical Exams (SHPE)
Who needs a Separation History and Physical Exam?
- All Active Duty (AD) and Active Guard and Reserves (AGR) Service Members
- Reserve Component (RC) on AD orders for 180 days or more of continuous service
- RC on AD orders for 31 days or more in support of a contingency operation
Service Members who are found unfit for duty through MEB are not required to complete a SHPE as the DES exam meets the SHPE requirement.
SHPE Timeline:
180 days prior to separation/retirement:
180 – 90 days prior to separation/retirement:
Less than 90 days prior to separation/retirement:
- Complete the SHPE as described below.
Less than 30 days from separation/retirement or final out/terminal leave date
- Preference is to complete part 2 of the SHPE less than 30 days from separation/retirement date or start of terminal leave date, whichever is earliest. The SHPE can be completed during terminal leave. The SHPE cannot be completed by SMRC after the SM has been separated/retired.
- If the SHPE has been completed greater than 30 days prior to separation/retirement date or start of terminal leave, the physical will need to be validated utilizing DD Form 2697 by your TMC provider.
- If your BDD claim has been completed and the exams are in the electronic health record (in JLV), the VA Separation Health Assessment (VA-SHA) or “general exam” can be validated by SMRC or your TMC provider and does not require the full SHPE.
How to Schedule a Physical Exam with SMRC
All physical examinations consist of two parts:
Part 1 physical exams are done on a walk-in basis: Monday – Friday, 7:15-9 a.m., up to 30 walk-ins a day.
Part 2 physical exams are scheduled at SMRC upon completion of part 1.
- Part 1 begins with a screening by a nurse to identify need for labs, immunizations, x-ray, hearing test, and vision test.
- Part 2 is the physical exam with a provider. PT uniform is preferred.
Same day appointments for parts 1 & 2 are available for SMs traveling from greater than 75 miles away. Please email the clinic at

Preparing for part 1:
Please complete forms 2807-1 and/or 2808, as required, at Medical Operations Data System (MODS), utilize the MHA Module (all forms are located under the SHPE tab). Forms can be completed prior to coming in for part 1; computers are available in the clinic.

For school physicals, if you wear glasses, please bring them to your appointment;
do not wear contacts.
Chapter physicals require a memorandum, from the First Sergeant or Commander, requesting the type of chapter. An escort of the same rank or higher is required for Chapters 7, 9, 10, 13, & 14 -12.
